Post-discharge care

When you have to leave hospital, you may sometimes feel that you still need that little bit of extra care at home until your fully back to your old self. Sometimes it may just be helping with the small things or with medication or bathing. At Eleven Sisters we can help you with this. Our experienced care workers will assist with an elderly person’s rehabilitation and offer expert care during the recovery period.

What is Post-discharge care?
